Home Care Packages
A Home Care Package (HCP) is a flexible in-home support option providing a wide variety of services and products. There are four levels available, each providing a different subsidy that can be used to fund services to meet your care needs.

Commonwealth Home Support Programme
A Commonwealth Home Support Programme (CHSP) package is entry-level home care for people who require a few hours of weekly support. A financial contribution towards your services is required, usually on a service by service basis.

Out of Hospital Care
Out of Hospital Care (OHC) is a six-week program that provides personalised support at home through a range of community services. Available in NSW, Out of Hospital Care incorporates ComPacks, Safe and Supported at Home and the End of Life programs.

Homeshare Program
The Homeshare program brings together older householders who could benefit from help and companionship at home. Available in Melbourne's North West, a home sharer should be prepared to lend a hand in return for affordable accommodation.

Linkages Program
The HACC-PYP Linkages program is available to Victorians aged under 65, or under 50 for Aboriginal or Torres Strait Islander people, who have complex care needs and would benefit from case management support.

Pricing and Usage Guide
Your Care Connect Client Adviser will be your personal contact for all your home care needs. They will visit you to discuss your needs and options, and work out a Care Plan with you to spend your package funds on the services you need and choose.
