It’s important you have a strong connection with your care provider— people you get to know and trust. When you choose Care Connect to deliver your Home Care Package, we will work hard to build a community of care around you that includes a dedicated Care Manager and a team of Service Coordinators and care workers.

Your local Care Manager will partner with you to learn about your needs, help define your goals, develop your care plan, and provide ongoing advice and guidance.
