Meet Darryl who lives with his wife Gwen near Lismore. He is a proud grandfather and enjoys spending time with his family and pet dog Sam. With the support of Care Connect and his Home Care Package, he lives at home with chronic medical conditions that impact his daily life.
1. The challenge:
Recently, Darryl experienced deterioration in his health which led to a hospital admission and surgery on his liver. Happily, his treatment was successful, but his recovery is complex, requiring the management of drainage tubes and some reduced mobility.
2. Solution without a Care Manager:
As Darryl was not able to seek the necessary additional care from their daughter to remain living independently at home, it is likely that Darryl may have had to explore options outside of independent living at least on a temporary basis.
