The Government offers Home Care Packages to support you as you age, so you can keep living at home. Our role, as a Home Care Package provider, is to make it easier for you to receive the care services and home support you need to do so.

Taking time to listen to your life story is how our Care Managers learn what you need, and what brings you joy. They use this shared understanding to cocreate a fully personalised care plan with you that makes best use of your funding or finances.

When you choose Care Connect to be your Home Care Package provider, we’ll appoint a dedicated local Care Manager—a caring professional who’ll be part of your life from then on. They’ll learn about your needs, establish your goals, develop a care plan with you, organise your services, and provide ongoing advice and guidance.

Your Care Manager will be a local—like yourself—as will the support workers and service providers they arrange on your behalf. Together, they’ll form a community of care centred on you—a community you can trust absolutely, thanks to our rigorous vetting process.

The Government offers four levels of Home Care Package funding. Depending on their assessment of your care needs, you’ll receive up to $59,593 each year if eligible—the equivalent of 8+ hours of weekly support.

Level 1.
Entry-level care tailored to deliver a little help with basic chores or perhaps transport.

Level 2.
The second tier of funding is designed to cater for those who require a little more assistance to maintain their independence at home.

An intermediate package designed to provide services and products that support day-to-day tasks in your home and in your community.

The most comprehensive package available designed to cater for more complex needs.
