To coincide with Dementia Action week, Care Connect care finder program is hosting an information session and mini expo to raise awareness of dementia. Together with local community organisations we will highlight the services and supports available for people living with dementia, and their carers in the City of Casey. Together as a community we can Act Now for a Dementia – Friendly Future.

Care Connect care finder program
The care finder program is funded by the Australian Government to help vulnerable older people navigate the aged care system and find support services to improve their quality of life. Care Connect care finders meet with you in person and can help with at any stage of your aged care journey; registering with My Aged Care, assessment support, accessing services for the first time and changing or finding new services or supports.
Alfred Health
Offer information, support and resources for carers and young carers (25 years and under) who provide ongoing unpaid personal care or support. Alfred Health provide Carer Gateway (Commonwealth funded) and Support for Carers (State funded) services.
Australian Multicultural Community Services
Learn about Australian Multicultural Community Services programs available including their Multicultural Dementia Care and Support Centres. Their Multicultural Dementia Care & support Centres make a positive impact for the caregiver and the individual.
City of Casey
Casey’s Link Work Program is a free service that assists residents or local organisation to access information and practical support to increase knowledge and connections. Link Workers provide in person, or over the phone support to people who need help navigating aged and disability systems and sourcing information and connections to maintain wellbeing.
Services Australia
The Financial Information Service (FIS) helps you make informed decisions about your finances. The FIS service is a free service available to all and can help you to make informed financial decisions on a range of topics including retirement planning and aged care fees.
Dementia Support Australia
DSA provides behaviour advice for those supporting people living with dementia. This advice focuses on the needs of the individual to ensure that our support is most effective. You can contact DSA by phone (1800 699 799) or web referral form (at www.dementia.com.au).
